Carry out all social-related activities, in order to ease the patient’s situation, to maximize his/her adherence to treatment and to improve his/her psychosocial condition.

RESPONSIBILITIES AND TASKS

•          Identify social needs of patients through a social assessment. Respond to the patient’s social needs, according to the scope of the project, by giving direct social support (as transport, food, legal advice,…) and/or refer to other social actors, in order to actively contribute to the improvement of their quality of living.

•          Contact patients to follow up the evolution of their condition when needed (home visits, defaulter tracing)

•          Participate in the creation of any material needed for the activities of social workers. Take care of social services facilities, materials and equipment in order to ensure continuity.

•          Mapping and regular update of social partners, local authorities, other NGOs, and governmental social services in order to improve social services scope and responsiveness.

•       Keep records, statistic and files updated, reporting any social activity, in order to prioritize and develop appropriate programs or projects.

MSF has started a project that will provide essential primary health care to at least 72,586 people, of which 24,028 of them are IDPs for the next two years, with an emphasis on reproductive, maternal and child health, the project will focus on improving access to quality preventive and curative health care services, lifesaving services to pregnant and lactating women and children.

The location of the project will be Sababoru. We are targeting the Darme HC as a core facility having 24/7 services with secondary health care capacity, as well as, 15 kebeles for outreach with a clear referral mechanism to 4 less equipped HCs and from them to Darme HC and further referral to Hawassa or Negele hospital. The targeted areas and population will be in phases. In April we target 6 kebeles, in June we start service at Darme HC and in September the rest of the 9 Kebeles.

The social worker will oversee the development of a general strategy of social support for beneficiaries assisted by MSF´s medical teams in Darme HC and in the community during the outreach activities. S/he will work closely with the medical staff and HP team to develop the patient satisfaction survey in order to:

•          To improve patient experience in Darme HC

•          Adapt patient’s rights material to local language and ensure that posters are clearly displayed in the Darme HC

•          Dedicate time to speak with each patient admitted in Darme HC (patient’s rights, Basic necessities, vulnerabilities and critical needs, access to health services,…

Within the outreach team, coordinate the same activities from the Health Extension Program in the area MSF will be doing outreach activities for social support and general protection
